The effect of treatment and drugs is inherently difficult to measure or predict. Consequently, detecting corruption or intentional misconduct (as opposed to honest mistakes or poor management) is a difficult task. Additionally, there is a lack of clear guidelines on how to deal with such issues when they are detected. And in many settings, small bribes are culturally and socially accepted, making it hard to draw the line between favors and gifts and criminal misconduct.
